Welcome to the Aurora Public Schools Suicide Risk Assessment Website.

You will find everything you need to complete a suicide risk assessment including resources to enhance your practice.

All of the suicide risk assessment forms (Non-Acute, Acute, Safety Support Plan, School Re-Entry) are meant to be filled out electronically. After you submit each part, you will be emailed a confirmation email, PDF of the form you completed, and directions to proceed to the next section.

**Required for all Acute and Retroactive SRAs**

This form documents the conversation with the student/family about higher level assessments or treatments received due to acute

This is the documentation of the plan discussed with the student and/or family that will help the student stay safe when they experience suicidal thoughts.

Templates for Safety Support Plans:

Safety Support Plans are completed to document the steps the school is doing to with the student and family to ensure safety. The actual plans that are shared with the student to remind what steps can be taken to keep themselves safe have no required format. These plans should be individualized to meet the students needs and abilities. Below are templates of examples that can be used.

Child Abuse Reporting Process

Always:

  1. Make verbal report ASAP. Note report tracking number and human service contact person.
    • Arapahoe County: 303-636-1750
    • Adams County: 303-412-5212
  2. Document on CPS form 1203 (hard copy OR new PDF below)- complete all areas of form & submit
    • Form 1203 (hard copy) can be ordered through APS Print Services
  3. *Fax completed form to APS Child Protection Fax # 303-326-1268
  4. *File completed form in secure file at school

(*During school closure, faxing to APS and filing at school can wait until schools reopen)

Submitting completed forms:

1. Download (DO NOT complete until downloaded!) new PDF form and electronically fill out new PDF form with fillable fields. Form can be used for both counties.

2. Submit form:

Submitting to Arapahoe County: Download and complete. Submit through the button in the lower right hand corner of the form titled "Email Completed Form". If the "Email Completed Form" link does not work, email to [email protected] .

Submitting to Adams County: Download, complete and email attached to [email protected] .

"Suicide, addiction, and depression have a very close and interconnected relationship. More than 90% of people who fall victim to suicide suffer from depression, have a substance use disorder (SUD), or both."

"Of all the factors associated with suicide, only depression (66% of people who fall victim to suicide are dealing with depression at the time) is more closely correlated than substance abuse. Individuals with a substance use disorder are nearly 6 times as likely to attempt suicide at some point in their life."
